Application for a One-Day Class C, Special Alcoholic Beverage License

Instructions

Read the below carefully, as there are several documents you should prepare to upload on the next step of the form.

Applications must be submitted at least 10 days prior to your event.
No exceptions.

All one-day licenses must be issued to three (3) people attending the event and over 21 years of age.
Rules and Regulations - Chapter 1. General Provisions (e) Licensee. "Licensee means any individual whose name appear(s) on a license issued by the Board, ...is, able to speak, understand, read and write the English language in a proficient manner..."

The three (3) people who are applying must provide the following:

• Full Name
• Title (Bride, Groom, Mother, Father, Member, Vice President, etc.)
• Phone Number
• Date of Birth (All three (3) applicants must attend the event and must be over 21 years old)
• Email Address
• Full Address (Street, City, State, ZIP Code)
• Signature
• All three (3) applicants must upload their government-issued photo ID (front only).

If there is to be entertainment at the event, please select type of entertainment (i.e. Band, DJ., Personal Electronic, or Other).

You must upload an invitation, an e-vite, announcement, ticket or a flyer as proof of details acknowledging the event.

You must upload proof of non-profit 501(c)(3) status showing your Tax ID # / Federal Number.

You must upload the contract with property owner, a rental agreement, or if the applicants own the property, a document with yourselves indicating that you are permitted to have the event at that location and date-time.

Costs per licensed day:

• Class C (Beer, Wine) - $25.00
• Class C (Beer, Wine, Liquor) - $50.00

Licenses are processed at the time of submission.
